REQUIREMENTS:

• Current CV


• Board Certification of Documentation of Board Eligibility in Internal Medicine or Family Medicine


• Documentation of Medical Degree


• Current, valid, unrestricted license from any state


• Current and valid copy of DEA license


• BLS Certification
