Loyal Source is actively seeking a motivated Physical Therapy Case Manager to place into a 3-month travel position open in Sheboygan, WI. 

QUALIFICATIONS:

• 3 years of recent PT Case Management experience required


• Exp. with Peds Rehab needed


• Bachelor's in PT


• PT License


• BLS from AHA
